Parts A and C are required annually for each scout and adult leader. Part B required for all camps participants; including cub scouts, adults and siblings. This form will not be returned to you. Please make a copy and retain for your records.

Tour Permit Application

This form is required when going on trips more than 50 miles away from your normal meeting location. Send to Committee Chairman or Cubmaster for their signatures (they will submit the form to NCAC).
